• Provides direct services to/for LSG clients as assigned in a professional, courteous and caring manner. • Assists with screening potential clients through LSG"s intake forms and process. • Accompanies Case Manager on initial home visits for intake and assessments. • Assists with recovery service plan delivery, including making referrals, checking in with client, and documenting service provision updates. • Ensures that any services provided to the client are documented in case notes. • Ensures all who need a particular service receive it in a timely manner. • Relays all requests and messages from the clients to the Case Manager promptly and accurately. • Submits to each Case Manager all documentation required to prove that services have been provided. This may involve copying original documents. • Maintains all records in an accurate, complete, confidential manner as assigned by the Case Manager. • Maintains personal and professional boundaries in the performance of the job. • Ability to set limits and maintain the helping role of the practitioner and to intervene appropriately to meet the needs of the persons served or other family members. • Participates in in-service training and conferences related to refugee services. • Participates in LSG functions. • Participates as a team player with staff. • Conforms with and abides by all regulations, policies, work procedures and instructions.
